**We have three eye doctors at your service. If you would like to be seen by a specific doctor for your appointment, please contact our office and request to be scheduled with the doctor of your choice.
If you are a new patient or your insurance provider has changed, please complete the insurance information form for the location you booked your appointment with.
After we verify your insurance eligibility, we will send you an email confirmation that your appointment time has been reserved. For more information on the insurances we accept click here.
Complete or update the Patient Portal with your information. If you are an established patient please contact us for an access code.
ON THE DAY OF YOUR APPOINTMENT:
Bring your prescription glasses or sunglasses with you
If you wear contact lenses please bring an unopened blister pack with you and wear them to the appointment
Please bring any previous eye-exam records if available
Please bring a pair of sunglasses to wear after your dilation
We recommend all patients receive a full eye health evaluation which includes dilation and Optomap retinal imaging
